-We provide delivery, setup and instruction on all equipment on the enclosed list
(within a 60 mile radius of Crystal Lake, Illinois)
-We agree to maintain all rental equipment in good working order
-We will prepare and file most insurance and Medicare claims for considered
reimbursement
